Please note that our preference is for you to come to our office and let us assist you with the documents. Coming to our office can actually save you time and grief. That being said, we also know that people like to know what is needed and some like to fill out the forms on their own. If you send in the documents electronically, you must follow up to ensure that we received the documents. Additionally, please note that the documents are sent from our web server via email and are thus not HIPAA compliant. You are encouraged to not send sensitive documents electronically. If you choose to send them electronically then you agree to not hold Goshen HELP responsible if they are captured by outside parties.
Step 1. Application
To receive assistance for any program through Goshen HELP you will be asked to complete our application. Please fill out all of the relevant boxes. In the Demographic area, there are codes listed to help you give us the right information.
Step 2. Photo ID
We require a photo ID to confirm who you are. Be ready to bring it with you to our office. If sending electronically we must be able to read the text on the ID. We do not recommend sending your ID electronically.
Step 3. Proof of Residency
We will verify your residency in Goshen County. Please bring a bill or other document that lists your current address.
Step 4. Proof of Income
We need either the last 30 days worth of pay stubs, benefit letters, or our Self Declaration Statement for everyone who is 18 or older in the household. Please only use the Self Declaration form if you have no income or a way of verifying your income.
Step 5. Other supporting documents
The following are categories of documents you will need based on the type of assistance you are requesting.
If you are requesting help with rent you will need the following:
- You must have our Rent Assistance form completed and signed by the landlord.
- Your landlord must have a w-9 on file. If not, they will need to complete a w-9 in order for us to make a payment on your behalf. We do not expect the landlord to give you the w-9. They may mail it in or bring it to our office.
- You must have a signed lease from your landlord.
- We must be able to confirm that your landlord has the legal authority to rent to you.
Goshen HELP requires an W-9 on file in order to pay rent. We do not expect the landlord to give the document to the renter. It may be mailed or dropped off at our office. If it is being mailed, you can let us know.
All other bills
Goshen HELP can only pay directly to the vendor that is owed money. You must bring us a current bill that shows the name of the vendor, the amount, your name and address, and the account number. If you are seeking assistance for work to be done, then we require an estimate.

One last reminder that we do not recommend sending your documents electronically. This should be used only as a last resort and it must be understood that the Goshen HELP can't be held responsible for documents captured by outside parties.
